{"id":2297,"date":"2025-04-03T20:37:08","date_gmt":"2025-04-03T18:37:08","guid":{"rendered":"https:\/\/protheseorthopedie.com\/?p=2297"},"modified":"2025-07-23T08:52:29","modified_gmt":"2025-07-23T06:52:29","slug":"congenital-hip-dislocation","status":"publish","type":"post","link":"https:\/\/protheseorthopedie.com\/en\/luxation-congenitale-de-la-hanche\/","title":{"rendered":"What is congenital hip dislocation?"},"content":{"rendered":"<p class=\"wp-block-paragraph\">As its name suggests, this is a congenital disease, meaning that it is present from birth.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This is an abnormality in the formation of the hip joint. This malformation is formed during foetal life, in the mother's womb.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The term dislocation means that the head of the femur is no longer located in its seat in the pelvis but is dislocated, literally \"dislocated\".<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">There are several types of congenital hip dislocation, depending on the anatomical position of the femoral head in relation to the pelvis and the severity of the injury. These are known as anterior, intermediate or posterior forms.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In the most severe forms, this is known as \"inveterate\" dislocation: when the head of the femur is completely dislocated outside the pelvis (acetabulum).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In the less severe forms, the head of the femur is not completely dislocated. It may be subluxated or in place but with a poorly developed pelvic bone. The bone of the pelvis (the acetabulum) is often scarred and underdeveloped. This is known as dysplasia of the acetabulum or dysplastic acetabulum. This is not a true dislocation.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In hip dysplasia, as in congenital dislocation, the hip functions under abnormal and deleterious mechanical conditions.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The joint wears and deteriorates prematurely, and total hip replacement surgery is often necessary in the long term.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Congenital dislocation of the hip also has muscular, ligamentary and capsular consequences that must be analysed prior to surgery.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">What are we suffering from?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The symptoms experienced are the same as those of ordinary osteoarthritis of the hip (coxarthrosis). Patients suffer pain in the groin, in the buttock or along the thigh and sometimes in the knee.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">They also complain of hip stiffness, creaking and a limp when walking.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The condition can affect both hips.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Finally, in congenital dislocations there is often a difference in the length of the lower limbs, especially if the disease affects only one of the two hips. We take this into account when fitting the hip prosthesis to correct this difference in length.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Lower back pain? Back pain?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Congenital dislocation often has an impact on the back, particularly the lumbar spine. This is especially true when it affects only one side. In these conditions, the lower limb is shorter on the side of the hip dislocation and the pelvis is oblique, tilted towards the side of the injury. This difference in length affects the lumbar spine.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Back pain should therefore be taken into account in this disease.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Lumbar flexibility and\/or stiffness must be analysed before considering surgery.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">When should a hip replacement be considered?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">There are no hard and fast rules.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A total or partial hip replacement may be considered when you are in daily pain and non-surgical treatment methods are no longer effective: when drugs (analgesics and anti-inflammatories), infiltrations and physiotherapy no longer relieve the pain.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">There is no age limit or threshold for a hip replacement. It is essential to analyse each person's individual situation: profession, leisure and sporting activities, family environment, etc.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Hip prosthesis<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The insertion of a hip prosthesis in hip dysplasia and congenital dislocation of the hip may require reconstruction to repair anatomical deficiencies. In these cases, we use bone grafts from the pelvis.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Hip replacement surgery for congenital dysplasia and dislocation requires very specific expertise. It is more difficult than surgery for ordinary osteoarthritis of the hip. This type of surgery is not performed in all orthopaedic surgery centres.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A specific technique for approaching the hip (trochanterotomy) is most often necessary. This provides excellent exposure of the hip joint and enables the hip prosthesis to be fitted in the best possible conditions.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This technique was described by Professor Marcel KERBOULL and is only taught in certain specialist orthopaedic centres in France.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">After the operation, specific instructions are given to the patient depending on the procedure performed.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A total hip prosthesis restores normal mobility to the hip joint, eliminates pain and restores independence when walking and in activities of daily living.
